The goal of the Health Care for the Homeless program is to provide health care for individuals and families who are currently struggling with homelessness.

Children ages 12 years and younger whose households use vouchers show improvements in education, employment, and income later in life. Outcomes for adolescents vary by gender. Females 10-20 years of age whose families use tenant-based vouchers to live in lower poverty neighborhoods experience better health outcomes while males of the same age experience worse physical and mental health outcomes. Additional research is needed to better understand and address challenges faced by adolescent males.
CPSTF finds societal benefits exceed the cost of tenant-based housing voucher programs that serve families with young children who are living in public housing, provide pre-move counseling, and move families to neighborhoods with greater opportunities.
Tenant-based housing voucher programs give many people access to better housing and neighborhood opportunities, both of which are considered social determinants of health. Because these programs are designed for households with low incomes, they are expected to advance health equity.

To assess the impact of affordable housing on health care outcomes in a low-income population who have experienced housing instability.
Costs to health care systems were lower after people moved into affordable housing. Primary care visits went up after move-in; emergency department visits went down. Residents reported that access to care and quality of care improved after moving into housing.

The program is focused on reduction of pain and improvement of function for arthritis patients unable or unwilling to attend small group ASMPs, which have proven effective in changing health-related behaviors and improving health status measures.

The goal of the HOPS program was to improve overall health status and academic achievement using replicable strategies.
The HOPS intervention helped students who qualified for free or reduced price meals both stay within the normal BMI percentile and score higher on their state math achievement test.

HealthTran has helped to improve healthcare access and long-term health outcomes, as well as reduced preventable hospitalizations and unnecessary emergency department visits.

Home vaccination for children behind in their immunization schedule is an effective and relatively cheap method of completing recommended vaccinations, and can be particularly beneficial for disadvantaged families.

The goal of the Hospital Diversion Initiative is to connect individuals who chronically utilize the inpatient and emergency rooms with outpatient care.
After three months in the program, participants showed a 66% reduction in ER visits, 68% reduction in inpatient hospital stays, a 72% decrease in homelessness, an 18% reduction in unemployment, and a 66% decrease in past 30 day arrests. More than 350 individuals have been served so far.
