Promising Practices
The Promising Practices database informs professionals and community members about documented approaches to improving community health and quality of life.
The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

Children ages 12 years and younger whose households use vouchers show improvements in education, employment, and income later in life. Outcomes for adolescents vary by gender. Females 10-20 years of age whose families use tenant-based vouchers to live in lower poverty neighborhoods experience better health outcomes while males of the same age experience worse physical and mental health outcomes. Additional research is needed to better understand and address challenges faced by adolescent males.
CPSTF finds societal benefits exceed the cost of tenant-based housing voucher programs that serve families with young children who are living in public housing, provide pre-move counseling, and move families to neighborhoods with greater opportunities.
Tenant-based housing voucher programs give many people access to better housing and neighborhood opportunities, both of which are considered social determinants of health. Because these programs are designed for households with low incomes, they are expected to advance health equity.

The goal of this program is to educate during every visit, to assist patients in developing a longitudinal personal record of medical history and care plans, and to provide a tangible way to engage patients in their own care. The overarching goal was better compliance, recognition of medication side effects, and improved adherence to specific and agreed upon lifestyle changes.
Sixty percent of patients participate in care plan tracking with a health notebook, and 80% percent of patients complete a prep form to help organize visits.

Healthy Love seeks to provide a safe, culturally tailored intervention for heterosexual black women to reduce their disproportionately high risk of transmitting and contracting HIV and other STDs. Healthy Love aims to encourage sexual abstinence, HIV testing, and receipt of test results; increase women's condom usage during vaginal sex with male partners; and reduce the number of women's sex partners and unprotected anal and vaginal sex with male partners. Healthy Love also seeks to improve HIV/STD knowledge, self-efficacy for using condoms, intentions to use condoms, and attitudes towards condoms.
Healthy Love increased participants' likelihood of using condoms, being tested for HIV, and receiving their test results. The intervention also reduced participants' self-described actions with male partners that can increase black women's risks for HIV infection.

The goal of Healthy Relationships is to reduce the risk of HIV transmission by people who know they are HIV-positive by reducing the frequency with which they have unprotected sex.
Healthy Relationships seeks to develop decision making and problem making skills to reduce the risk and transmission of HIV through behavioral intervention. An intervention study resulted in significantly less unprotected intercourse and greater condom use with lower estimated HIV rates post-intervention.

The initiative's primary purpose was to reduce infant mortality by 50 percent and generally improve maternal and infant health in at-risk communities.
20% of the Healthy Start program sites had significantly lower rates of low-birth-weight babies than their comparisons. 20% of the sites also had significantly lower rates of very-low-birth-weight babies than their comparisons. Four of the sites had significantly lower pre-term birth rates.

The Community Preventive Services Task Force (CPSTF) recommends self-measured blood pressure monitoring interventions combined with additional support to improve blood pressure outcomes in patients with high blood pressure. Additional support may include patient counseling, education, or web-based support. Economic evidence indicates that self-measured blood pressure monitoring interventions are cost-effective when they are used with additional support or within team-based care.
